This form is used to provide an overview of the courses to be taken as an AMCB student and help determine the course requirements and expectations of the supervisory committee.

Annual Student Evaluation
Each student is required to complete an annual evaluation with the chair of their supervisory committee. Annual evaluations are due June 30 each year.
